The past perfect of modal verbs is always formed with the auxiliary verb haben in the Präteritum and the past participle (Partizip II).
The past participle (Partizip II) is formed with the prefix ge, the present perfect stem and -t, the ending of regular verbs.
Note: Only in the case of sollen and wollen are the present perfect stem and the present tense stem identical.

Note
When modal verbs are combined with an infinitive, the present perfect and the past perfect tenses are not formed with the past participle of the modal verbs but with the use of that particular infinitive. See more at "Special ways of perfect tenses formation".
